Again, probably thinking about this too much. But isn't there a difference between saying 'he's mental' and 'he's a mental', which makes the latter sound like stigmatising or at least differentiating a group.
'Dave's black' or 'Dave's a black'. 'Dave's gay' or 'Dave's a gay'.
I think there is a distinction, and if I were Dave I'd be bristling at the definite article.
Unsurprisingly, I think I saw something about this sort of thing on a Guardian language blog a while ago...

For me, Ricky Gervais is still in credit at the Bank of Comedy because the Office was so good, and Extras was OK too. The dwarf stuff is a bit rubbish.
So, the letter "a" can cause offence (though I take your point, DrT)
That's why rdvrks are so unhappy.
I feel I have to wade into this debate for no other reason than I don't find either word, "mental" nor "twat' offensive.
Simply because I am neither.
